Introducing the Integrated Services Delivery Model (ISDM)
Abstract
An overview of the new Integrated Services Delivery Model (ISDM) for the Calgary Bridge Foundation for Youth (CBFY). IDSM coordinates services from the Connect Team, the Bridge Link Team, and the NEST Teams.

Key findings:
1) A triple Venn diagram indicates the overlap and differences between the Connect Team, the Bridge Link Team, and the NEST Teams.
2) The Connect Team at the Welcome and Reception Centres initiate the case management process, process onboarding, refer to other parts of CBFY, and facilitate other communication and appointments.
3) The Bridge Link Team in the Northland building welcomes families and completes NAARS intake, including information sharing, referrals, connecting to supports, community building, and ongoing case management.
4) The NEST Teams (Neighbourhood Newcomer Engagement in Schools Team) provide school clusters of support. They work in both the public and Catholic school boards with individual support, collaborating with other school supports, mentorship, interpretation and ongoing case management.
